SIGN UP FOR THE QUARTERLY NEWSLETTER AND DRAWING HERE!!!

There are a few rules/limitations for the contest as follows:

1. Winners will be selected in a random drawing from all persons signed up to receive the quarterly newsletter.  Drawings will be held once per quarter on a date determined by Dr. Mills, and will be conducted by Dr. Mills or a staff member.  A selected patient will witness the winning drawing.

2. Winners must have had a minimum of an initial dental exam, cleaning and x-rays to be eligible.  This is for our insurance purposes.  Winners may schedule their initial exam/cleaning/x-rays at the time the prize is claimed.

3. All winners will be notified by email, using the email address on file.  Dr. Mills cannot be responsible for invalid email addresses or otherwise undeliverable emails.  Winners must claim their prize via telephone within ten (10) days of drawing.  After ten days, an alternate winner will be selected.

4. Only one email address per patient is allowed.  Multiple entries will invalidate any winning drawing.

5. Winners may not have won any other prize or drawing from Dr. Mills in the past twelve (12) months.

6. Eligibility and winning status is at the sole discretion of Dr. Mills.  All decisions are final.
